Arrive early with your dental and medical history
You will be required to fill in a form outlining your dental and medical history. This takes time so please arrive at least 15 minutes before the appointment. We will need to know about your dental insurance, family doctor, medications and more. It is a good idea to look at the form before coming, even if you choose not to fill it out ahead of time. There may be some items that we need to clarify so always come early.

Do you direct bill for insurance?
Yes, for most treatments. We can direct bill your insurance so that you do not have to pay first, and submit later. Please make sure to bring your personal insurance information with you to your appointment, and notify our administrative team if you have any changes.
Do you accept the Canadian Dental Care Plan (CDCP)?
Yes, we are accepting CDCP coverage for patients. Be sure that you’ve ensured your coverage information is up to date and accurate prior to coming in for your appointment.
How often should I get a cleaning?
This depends on your individual treatment needs, and is best answered by our knowledgeable dental hygienists, following an initial assessment and treatment. We see a diverse range of patients, and try to tailor specific treatments for each individual, utilizing hand scaling, ultrasonic debridement, chemotherapeutics and referrals to dental specialists when appropriate.
